Registration Fee: $199.00 per team (U9 thru U15 Youth Teams)         
Registration Deadline: Register early as space is limited. Registration will close when full.

  • Team Check-in Instructions: ONLY 1 PERSON NEEDS TO CHECK IN THEIR TEAM.
         All teams are required to check in on Thursday, February 9, 10 am to 7pm.
  •  at the NWOYSL office, located at135 Chesterfield Rd Suite 104.,Maumee, or you can register at the facilty and will need to report at least 1½ hours prior to your first scheduled match.  This will be the only time to check in your team for the tournament. 
  • You will be required to provide the following for the registration on February 9th:
  • Two (2) typed copies of your Official Roster, including jersey numbers. (The rosters must be properly validated and cannot exceed the maximum player or coach requirements.)
  • Current player passes. (The passes must be properly validated, and contain pictures and signatures of all players and coaches.)
  • Notarized Medical Release Form (www.festivalsoccer.org) for each player, including guest players.
  • Team Liability Release Form, signed by all players families.
  • Weekend Contact Form
  • Guest Player Approval from our forms page below (The rosters must be properly validated and cannot exceed more than three.)

FOR ALL TEAMS:


 
  • GUEST PLAYERS FORM from MULTIPLE TEAMS..."for example: three players from one team, two from another team, 4 from another team.  Different Clubs can be combined if they are all the same age level.
